As of now, the entry form for Spain (Spain Travel Health) does not need to be filled out as long as you have a digital EU Corona certificate that proves via QR code that you have been vaccinated, recovered or tested.
Those who do not have the QR code must still fill out the entry form.
However, nothing has changed in the entry requirements. For entry into Spain - and thus also to Mallorca - still applies: Only vaccinated, recovered or tested.
